Program Summary

Project Name: Better Together Hennepin: Healthy Communities Healthy Youth Project (HCHYP)

Project Overview: Hennepin County Public Health’s Better Together Hennepin: Healthy Communities Healthy Youth Project (HCHYP) will expand on its award-winning Health Mentor Model so that young people with the greatest needs will receive the education, support, and encouragement they deserve. In the Health Mentor Model, Health Mentors provide programming to students in one-to-one, group, and classroom sessions, ensuring every young person receives health education and support in a manner that works for them. Health Mentors also address systems at their site to make it as easy as possible for young people to be healthy. HCHYP will place Health Mentors in multiple settings across Hennepin County that serve communities and populations most negatively impacted by health inequalities, including 12 high schools, 11 adolescent-friendly clinics, and the Hennepin County Juvenile Detention Center. HCHYP aims to serve 28,760 young people over five years.

Community(ies) Served: High schools, clinics, and juvenile detention centers in specific neighborhoods within Minneapolis, MN (Brooklyn Center, Hopkins, North Minneapolis, Central Minneapolis, Richfield, and Robbinsdale)
