Program Summary
Project Name: Wisconsin's Advancing Equity for Adolescent Health through Evidence-Based TPP Program
Project Overview: Through this funding opportunity, the Wisconsin Adolescent Health Team proposes to build upon existing and developing new partnerships to implement evidence-based youth programs with priority youth in Wisconsin to address health disparities and provide youth with the knowledge, skills, and supports they need to thrive. The Adolescent Health Team will support high quality implementation of evidence-based programs in three unique setting types – community-based organizations, schools, and system-involved youth residential and service settings – in order to reach youth of the highest need. Long term goals of this project include decreasing unintended youth pregnancy rates, reduce disparities of sexually transmitted infections (STI) rates and teen pregnancy rates among Wisconsin adolescents, and increase number of youth who feel connected to their community and youth-friendly services. These services aim to reach 1,750 youth.
Community(ies) Served: Community-based, school, and system-involved youth settings in priority counties in Wisconsin
Target Population: Priority populations include youth with disabilities, youth involved in the child welfare and youth justice systems, Black, Hispanic, and indigenous youth, and LGBTQ+ youth.
Geographic Area Served (Urbanicity of Community): Counties in Wisconsin with teen pregnancy rates in the top 2 quintiles (top 40%) for teen pregnancy rates and/or with top 2 quintiles (top 40%) for teen STI rates (Urban/Rural)
Average Anticipated Number of Youth Served Annually: 1,750
