Project Name: Trinity Church PlanBe_ Teen Pregnancy Prevention Project

Project Overview: Trinity Church, Inc. and its partners will implement the PlanBe_ Teen Pregnancy Prevention Project using the proven effective Love Notes 3.0 and Reducing the Risk curricula. The project will empower youth and their caregivers with the skills needed to decrease teen pregnancy and sexually transmitted infections (STIs) and achieve optimal health through teen pregnancy and STI prevention education, events, workshops, curricula homework, email blasts, a social media campaign, support services, mental and emotional health education, and human trafficking community workshops. The project will also utilize the Trinity Community Advisory Group and Youth Leadership Council to ensure adequate stakeholder representation, oversight, and expertise of project implementation. The project aims to serve 5,350 youth and 3,750 families/caregivers annually.

Target Population: Youth in middle schools, high schools, foster care, group homes, and pregnant and parenting educational programs, as well as those involved in the juvenile justice system and living in areas with high levels of human/sex trafficking; caregivers
